Request for Grant Revision Form
A Grant Revision Request Form needs to be submitted immediately if you foresee changes to your NMCF grant. Grant revisions include:
- Need for a change/extension to the grant period
- Change in scope of the project
- Changes to your grant’s approved budget
Your request will be reviewed by Denise Gonzales, NMCF Director of Community Philanthropy, and other NMCF program officers, as appropriate. In some cases, we will also consult NMCF funding partners prior to providing a final determination for your request. Please note that approvals are not automatic.

NMCF will acknowledge receipt of your grant revision request and will provide a final decision to you in writing within 20 days of receiving your receipt, unless additional information is required for us to make a decision.
